Being “Aware of your surroundings” is not as easy as it sounds.
There are many medical and mental reasons why many children and adults are not physically nor mentally able to be observant, to pay attention, or even to really care about their surroundings. People with conditions like ADD, ADHD, Bipolar Disorder, Depression, Diabetes, Fetal Alcohol Spectrum Disorder, and Panic Attacks come to mind.
They are very much at risk when it comes to personal safety.
